Form Packets
General Instructions
The Court has assembled the below "Packets" of forms and informational materials to assist self-represented litigants with filing certain types of cases. Each packet contains the forms most commonly filed in each case type, along with some applicable reference information regarding the process and various related Court programs. Packets can be purchased in hardcopy at the Courthouse, or can be downloaded here at no cost.
If you opt to download the Packet, it will download as a ZIP file containing individual fillable PDF forms along with the information materials (the one exception is the Civil Alternative Dispute Resolution packet, which will download as a non-fillable PDF file). These files can then be filled out electronically, printed, and signed for filing with the Court. Documents can either be filed at the Courthouse at 1775 Concord Avenue, Chico, CA or can be filed electronically. For more information about electronic filing, click here.
